Platinum Tax Service Checklist (Tax Amendments)

Fax (Recommended), Mail, Scan or Send through eSupport- the following documents that are applicable to you. Please understand that you will NOT have all of these documents.

If you plan to mail us the Tax documents, please take photocopies of the original documents and mail us the photocopies of those documents.

• Please Fill and Submit the Silver Tax Form.


• Visa and I-94 Pages for Yourself and your Spouse (if applicable) from the Passport.


• Visa and I-94 Pages for your child(ren) (if applicable) from their Passports.


• Tax Return being amended.


• All W-2(s).


• Forms 1099-MISC (from employer to document payments to you as an independent contractor).


• Forms 1099-INT (from bank, savings account, credit union).


• Forms 1099-DIV (from brokerage account, mutual funds, etc.).


• Form 1099-B (from brokerage account, mutual fund, etc.). This includes any end-of-year statement that shows all stock trade transactions.


• Form 1099-G (for unemployment compensation and state tax refund).


• Forms 1099-R (from banks, brokerage houses, mutual funds, 401K custodians, etc. to document distributions from pension plans and IRAs)

This list is not comprehensive, but represents the forms that are most commonly needed to prepare an amended tax return.
