Checklist to File Form 1040Fax (Recommended), Mail, Scan or Send through eSupport- the following documents that are applicable to you. Please understand that you will NOT have all of these documents.
If you plan to mail us the Tax documents, please take photocopies of the original documents and mail us the photocopies of those documents.
- Last Year's Tax Return (Optional, but highly recommended)
- All W-2(s).
- Forms 1099-MISC (from employer to document payments to you as an independent contractor).
- Forms 1099-INT (from bank, savings account, credit union).
- Forms 1099-DIV (from brokerage account, mutual funds, etc.).
- Form 1099-B (from brokerage account, mutual fund, etc.). This includes any end-of-year statement that shows all stock trade transactions.
- Form 1099-G (for unemployment compensation and state tax refund).
- Forms 1099-R (from banks, brokerage houses, mutual funds, 401K custodians, etc. to document distributions from pension plans and IRAs)
- Forms SSA-1099 (from the Social Security Administration to document the social security benefits received)
- Form 1098 (for mortgage interest paid on personal homes and rental properties).
- Forms 1098-T (from colleges/universities to document tuition paid and scholarships received).
- Schedules K-1 from partnerships, S corporations, trusts.
- If receiving alimony, amount of alimony received for 2007.
- Tax statements from your City/County/State.
- Receipts for charitable donations, both cash and non-cash.
- Receipts for medical expenses (will be deductible only if it exceeds 7.5% of your Adjusted Gross Income).
- Student loan interest statements from Sallie Mae or banks for interest paid on student loans.
- Annual statement from daycare provider showing amount paid annually to daycare. Statement must have name and address plus tax identification number of the daycare provider.
- Closing statement from house purchase (HUD Form 1).
- Proof of wages/salary received by taxpayer or spouse while still in your home country.
- Health savings account statement.
- All income and expenses associated with rental real estate.
- If paying alimony, name and SSN/ITIN of recipient.
- Proof of health insurance payments.
- Proof of contributions to IRAs, SEPs or other qualified pension plans.
- Proof of alimony payments received or paid.
- Proof of purchase of the hybrid vehicle for Alternative Motor Vehicle Credit.
- Receipts for expenses associated with the adoption of a child.
- Receipts for improvements to home that qualify for the Resident Energy Credit.
- Proof of foreign taxes paid.
- Receipts for moving expenses.
- A VOIDED check from the Checking or Savings account where you want the refund deposited or the tax due withdrawn.
